The book has taken on a life of its own, and has grown tremendously and proportionally to newly uncovered material. The book has far over 2,000 pages after the first 2,000 references. There are over 7,000 other references that are waiting to be integrated into the main text. A shorter version of the book which provides a summary of the key data will also be published.
